If you are responsible for health reporting at a school or childcare facility in Van Buren or Cass County, use this page to:

Submit Your Weekly Report

Schools and childcare facilities should submit a weekly communicable disease report, even when there are no illnesses to report.

When to Contact the Health Department

Do not wait for your weekly report if you have a serious communicable disease or a cluster of illness.

Contact the Communicable Disease team as soon as possible if:

  • A serious or reportable communicable disease is suspected or confirmed.
  • You notice an unusual number or cluster of illnesses.
  • Your school or childcare facility closes because of illness.
  • You are unsure whether an illness needs to be reported.

Early reporting allows public health staff to investigate potential outbreaks, provide guidance, and help prevent the spread of disease.
